Camper Details Form

Thank you for your purchase! Now we just need a few details.

An individual form needs to be completed for each camper. Please select either 'Families Week Camper Form' or 'Youth Week Camper Form' to continue.

Once you've finished the form, please click 'Submit' and then you'll be able to add additional campers. Each form will take approx. 5-10 mins to fill out. You can save and resume the form at a later date at the bottom but please make sure it's filled in as soon as possible to help us prepare for camp!

If you've donated to MWCC please ignore this form, and head over to our Gift Aid page.

To comply with the Data Protection Act 1988, we need your permission before we can photograph or make any recordings of your child for promotional purchases. Please note that website can be viewed throughout the world and not just in the united kingdom where UK law applies.
